In blepharoplasty operations, eyelids should be evaluated together with the structures they are adjacent to. Lower eyelid should be evaluated together with cheeks and the upper eyelid should be evaluated along with eyebrows and it is preferred to move the fat pads under the cheek, if there are any in the lower eyelid. The eyelid aesthetic creates a change that will enhance one’s self-confidence as a result of the revival, removal of the tired expression and the younger appearance. The eyelid aesthetic leads to improvement and rejuvenation in facial expression, which looks sad, tired and angry, as well as with vividness in the eyes.
If blepharoplasty surgery is combined with eyebrow lift, forehead lifting, temporal area stretching operations in cases where the low eyebrows and wrinkles on forehead exist, and if it is combined with mid-face lifting (cheek lifting) when saggy cheeks in question, the aesthetic result is much more satisfactory.
Surgery is done by entering from the fold line in the upper eyelid, and 2 mm below the eyelashes in the lower eyelid, eye lid and surrounding cuts on the skin have the best healing capacity in the human body and there will be no scar after eyelid surgery in this area.
The skin with excess wrinkles and sagging in the lower eyelids are stretched, and if there is bagging problem in the same person, a new look is provided by giving a new position to the fat pads that causes this bagging. Fat pads are dealt by a couple ways;
This is a method, which is not used frequently nowadays but it is rarely applied to over-weighted persons. In these surgeries which are made by taking the fat pads, the eye bowl retreats, the wrinkle is reduced but the pitched and aged gaze comes out. Another approach is the placement of herniated fat pads to their former places and the application of plication to orbital septum (membrane structure in front of the fat pad). This method is mostly applied to young patients who have strong orbital septum.
Subperiosteally fat redistribution is another method that can be applied to a great majority of cases, which smooths the transition of the eyelid to the cheek, contribute to the volume of the cheek, and in which the fat pads contributing the cheek volume are placed to under the cheek texture.
Ophthalmic aesthetic surgeries are usually performed under local anesthesia if there is no additional intervention. I usually perform lower eyelid surgery under general anesthesia, and upper eyelid surgery under local anesthesia. Average upper eyelid surgery takes 30 minutes and lower eyelid surgery takes 1 hour. Postoperatively, there will only be thin bands on the eyelids and the eyes will not be closed, pain does not usually occur, edema and bruising will decrease within an average of four to five days, sutures will be removed on the seventh day of five days and the patient will be able to continue with daily routine life. Our final result becomes clear after the second and third month.
